Visiting Nurse Services

Patients who need home care will be referred to Visiting Nurse Services agencies. Care is coordinated through the hospital nurse case manager. The nurse case manager communicates treatment plans and special needs that are coordinated by the Visiting Nurse Services agency upon discharge.
