To obtain copies of your medical records from Bridgeport Hospital please:           

1) Print and fill out the Medical Records Request Form.           

2) Mail the signed form to:               

After you send in the form, you will receive a bill (65 cents a page) from Healthport. Once they receive your payment the medical record will be sent to you.
