2007 BENEFITS SUMMARY
Regular employees scheduled to work 24 or more hrs. become eligible for medical, prescription drug, dental, vision and life insurance the first of the month following 90 calendar days of employment. Employees are eligible for Short Term (STD) and Long Term Disability (LTD) effective the first of the month following 1 year of benefit eligible service (Part Time employees are not eligible for LTD).
BENEFIT PLANS
Medical Plan Options: (Administered by Anthem Blue Cross & Blue Shield – No referrals required
Yale New Haven Health Advantage Plus Plan provides in- and out-of-network coverage.
- Highest level of benefits when services are provided at a YNHHS hospital.
- In-network benefits: $20 copay office visits and 100% coverage for most services. No claim forms required.
- Out-of-network benefits: 70% coverage for many services after $250 individual/$500 family deductible is met.
Yale New Haven Health Advantage Plan: Lower per pay contributions, but must meet a deductible if you need major health care services.
- In-network benefits: Preventive care covered at 100% after $20 copay, no deductible. Physician/ specialist office visit $30 copay, no deductible. All other services covered at 80% after $200 individual/$400 family deductible.
- Out-of-network benefits: $750 individual/$2,250 family deductible; 60% coverage for most services.
Prescription Drug Program: This plan is included with your medical plan election and is administered by CaremarkPCS. Employees pay, per prescription, 10% ($8 min/$23 max) for generic drugs, 20% ($20 min/$45 max) for preferred brand drugs and 40% ($30 min/$60 max) for other brand drugs at participating pharmacies (no coverage at out-of-network pharmacies). Up to 30-day supply provided. Mail order available for maintenance drugs at $16/$40/$60 co-pays for a 90-day supply. Employees taking maintenance drugs must use mail order.
Dental Plan Options:
- Delta Dental Basic Plan covers 100% of preventative (no deductible) and 80% of restorative services after a $50 (single)/ $100 (family) deductible.
- Delta Dental Plus Plan covers 100% of preventative (no deductible) and 80% of restorative services, 50% of major services, 50% orthodontic services after a $25 (single)/ $50 (family) deductible.
Opt-Outs: Employees who have medical coverage through another group plan and are electing to waive our medical coverage are eligible to receive annual Flex credits of $525 (full time) and $425 (part time). If your spouse is covered by another YNHHS medical plan, or you are covered under Title 19, you may not receive the opt-out payment.
Vision Service Plan (VSP): Benefit eligible employees can enroll in the Vision Plan. VSP network doctors provide top quality care and offer a wide selection of frames to choose from- all in one convenient location. Get exams, contacts, frames and/or lenses at affordable prices.
Flexible Spending Accounts: Benefits eligible employees may elect to have a portion of their salary deducted on a pre-tax basis, to pay for medical and/or dependent care expenses. Use of these salary conversion dollars reduces the amount of gross income subject to income taxes and social security.
Life Insurance: Benefit eligible employees are enrolled in life insurance equal to one times their annual salary (to a max of $50,000) at no cost. Employees also have the option of purchasing with post-tax dollars, additional insurance up to five times their annual base salary (to a max of $500,000). Spouse & child life insurance is also available.
New Hire Benefits: Bridgeport Hospital provides access to medical coverage for new hires during the enrollment waiting period for this plan. Contact the Benefits Department for more information.
SALARY CONTINUATION PLANS
Short Term Disability: Benefit eligible employees may enroll in the short term disability program effective the first of the month following one year of benefit eligible service. The STD Plan provides a bi-weekly benefit of 60% of base salary. Sick time must be exhausted before this benefit is paid. Sick and STD combined can be paid for no longer than 6-months. If an employee expects to be out of work for more than 7 calendar days due to maternity, illness or injury, they must contact Aetna at 1-800-488-2386 to evaluate and certify their length of disability.
Long-Term Disability Program: Full time employees may enroll in the long-term disability (LTD) program effective the first of the month following one year of benefit eligible service. The LTD Plan provides a monthly benefit of 60% of base salary following a 6-month qualification period.
RETIREMENT SAVINGS PLAN
Bridgeport Hospital offers a Retirement Savings Program (RSP) to employees. This program offers a convenient, tax-advantaged way to save for retirement. Employees must complete one year of eligibility service to qualify for the automatic annual Hospital contribution. Your automatic annual Hospital contribution is based on total pay and years of credited service. Employees must be employed on December 31 of the calendar year and complete at least 1,000 hours of service during that year to receive the automatic annual Hospital contribution for that year. Employees may also enroll at any time to contribute to a TSA/403(b) account and select their investment options. The Hospital helps employee contributions grow by matching their TSA/403(b) contributions each pay period. IRS limitations may apply to employees determined to be highly compensated. Please refer to your enrollment guide (mailed to new hires’ within the first month of hire) for full plan details and the “Next Steps” to enroll in the plan.
New Hires: Unless they elect otherwise, the Hospital will deduct and contribute 2% of the new hires’ base pay to a TSA/403(b) each pay period after their 60th day of employment. The Hospital will also make a matching contribution equal to 1.5% of the new hires’ base pay for each of the same pay periods. This will continue unless the employee calls Fidelity to make a different election.
WORK/LIFE BENEFITS
Legal Holidays: Bridgeport Hospital offers 6 Legal holidays per year: New Years Day, Memorial Day, 4th of July, Labor Day, Thanksgiving and Christmas.

Floating Holidays: Bridgeport Hospital offers 6 Floating holidays in a calendar year. Employees are eligible to use 1 Floating Holiday every 2 months.

Vacation Time: Employees accrue 2 to 5 weeks of paid
