What is REACH?

Bridgeport Hospital’s REACH Program is an intensive outpatient program (IOP) for children, adolescents, adults and seniors.

The REACH Program provides intake assessments, medication management, group therapy, case management and after care planning.

REACH is located at 305 Boston Avenue in Stratford. For more information about our services, call 203-384-3377, 8:30 a.m.-5:00 p.m., Monday-Friday.

The emotional, behavioral and psychiatric needs of adolescents and their families are unique and are best cared for in a specially designed treatment program.

The REACH Adolescent Program is an afternoon intensive outpatient program (IOP) for adolescents between 13 and 17 years who are experiencing emotional and behavioral difficulties.

The Program is designed to reduce the need for hospitalization, decrease residential placement and assist in keeping families intact. We offer the following services: assessment, group therapy, multi-family group, art and pet therapy, medication management and case management.

Who May Need Our Help?

Adolescents who exhibit the following symptoms may benefit from treatment:
  • depression or social isolation
  • suicidal thoughts or passive death wish
  • extreme mood swings
  • aggressive, threatening behavior
  • hyperactivity
  • school avoidance or refusal
  • oppositional behavior

Adolescent Program Hours

Mondays, Tuesdays, Wednesdays and Thursdays: 1:30 p.m.–4:30 p.m.

Families are required to attend the weekly multi-family and parent support group.

Our Staff

The REACH Adolescent Program is staffed by a multidisciplinary treatment including social workers, an advance practice psychiatric nurse, psychiatric technicians, an art therapist and support staff. Additionally, as part of Bridgeport Hospital and a member of Yale New Haven Health, the REACH Adolescent Program has ready access to a variety of other medical and rehabilitation specialists and nutritionists.

How to Access the Program

Patients may be referred by schools, clinics, physicians, the Department of Children and Families (DCF) and other healthcare providers. Parents may also make direct referrals to the program. Please call 203-384-3377, 8:30-5:00 pm and ask for an evaluation appointment.

Insurance

The REACH Adolescent Program is covered by Medicare, Medicaid and most commercial insurance plans.
