The REACH Adult and Older Adult Program, a part of Bridgeport Hospital’s Department of Psychiatry, is a community based half-day Partial Hospital and Intensive Outpatient program designed to meet the emotional needs of adults and older adults and to reduce the need for hospitalization.
Patients are place in one of four separate tracks designed specifically for their individual needs. These include a track for patients with emotional and substance abuses issues, a track to address the unique needs of the older adult and a track for the acute and the chronically mentally ill adult.
The REACH program is staffed by a team consisting of a psychiatrist, psychologist, social workers, psychiatric nurses, therapeutic activity therapist and support staff.
Service are covered by Medicaid and most commercial insurance plans. Additionally, as part of Bridgeport Hospital (a member of Yale New Haven Health), the progam has ready access to a variety of other medical and rehabilitation specialists and nutritionists.
Patients may be referred by physicians or other healthcare providers. Patients may also call the program directly for an evaluation appointment.
