YNHHS goal: A high reliability organization

YNHHS is in the midst of a multi-year project to become a high reliability organization (HRO) and eliminate incidents of preventable patient harm. HROs are organizations that, despite working in high-risk fields, have substantially reduced errors by implementing specific safety measures and training all staff on error-prevention techniques.

At YNHHS, the HRO effort is being phased in at each of the hospitals and NEMG. Bridgeport Hospital, along with the other YNHHS hospitals, has completed a major step in the HRO process, analyzing past serious safety events to determine what specifically went wrong and how the errors that led to these events could be prevented.

The next step is training on safety behaviors specific to issues at YNHHS. These behaviors focus on clear communication, effective patient handoffs, mentoring others and practicing with a questioning attitude. Training is expected to begin later this winter for YNHH and Greenwich Hospital employees and physicians, and is scheduled to begin in the spring or summer at Bridgeport Hospital.
