If a patient would like to execute an advance directive (living will) or a power of attorney, please contact Patient Relations at ext. 3704 during normal business hours, Monday–Friday.

If you need information about advance directives, please ask the Admitting Office for a copy of our brochure, "What You Should Know About Advance Directives, Including Living Wills."
