Advance Directives
Patients who need information about, or would like to execute, an advance directive (living will, and/or designation of healthcare representative) are invited to speak with the caregivers on their unit.
Notary Services
Notary public services for patients are available free of charge by appointment. Please call Patient Relations, at extension 3704 from inside the hospital, Monday – Friday, 8:00 a.m. to 4:30 p.m. Please note that services are provided at the discretion of the Notary.
