Postpartum Wellness Check Program
Before you are discharged, we invite you to schedule a post-partum wellness check visit, where a specially trained nurse practitioner will assess the health and well-being of you and your baby. The visit will be scheduled 3 - 7 days after you are discharged to identify potential problems early.
Your appointment may be a video visit and will require internet connection and MyChart app access. An In-person visit will be scheduled if needed at Bridgeport Hospital.
What to expect at your visit
Our nurse practitioner will check:
- Mom’s blood pressure
- Wound healing, if appropriate
- Mom’s emotional well-being and stress levels
- Baby’s weight and feeding pattern
Our nurse practitioner will also answer your breastfeeding questions and, if needed, will direct you to a lactation consultant.
Your visit will be scheduled before you are discharged and any costs associated with the visit will be discussed. Your visit may be covered by your insurance.
