Frantzie Fequire, PCT, West Tower 7, earned the Great Catch Award for her actions after a patient returned from an angiogram. While assisting the primary nurse with routine care, Fequire noticed significant bleeding from the patient’s groin access site. Fequire immediately spoke up, and the care team responded quickly, applying manual pressure until a surgical resident arrived and achieved hemostasis.
Elena Buglione, RN, Surgical A, earned the Great Catch Award after noticing that a patient’s assigned nurse had stepped away after initiating a blood transfusion. Buglione respectfully reinforced the importance of remaining with the patient during the required 15-minute monitoring window for blood transfusions. Buglione stayed with the patient during that window, and her vigilance and advocacy helped prevent a safety risk.
John Cruz, OR assistant, Saint Raphael Campus, was recognized for attention to detail and practicing a questioning attitude. While bringing a specimen to Pathology, Cruz saw no label on the specimen or requisition form in the specimen bag. He escalated his observations to the charge nurse, who verified with the OR staff. Real-time education was provided to a new traveler nurse on how to prep and send a specimen to the Lab. The nurse appreciated the cross-check and 200% accountability, and the specimen was sent to the Lab and processed without delay.
Kristen Cocca, RN, SRC Pre/PACU, was recognized for attention to detail, 200% accountability, practicing a questioning attitude and speaking up for safety. While caring for a patient scheduled for spinal fusion surgery, she saw the care team preparing the patient for a pre operative block. Cocca requested a pause to validate and verify the surgical consent. When a review showed the consent did not match the intended procedure, Cocca stopped the line and escalated her concern to the surgical team. The team confirmed the discrepancy and obtained a corrected consent, allowing the block and surgery to proceed.
Sarah Lamsam, RN, York Street Campus Adult Emergency Department, was recognized for attention to detail, practicing a questioning attitude and speaking up for safety. While processing a patient in triage, Lamsam noted that the visitor accompanying the patient had entered the building with a weapon. The weapon had been detected and secured according to protocols, but after the patient was admitted to Labor and Birth, the visitor returned to retrieve the weapon and requested access to the unit. Lamsam directed the visitor to the front entrance (where they would be screened for weapons) and immediately notified Labor and Birth to ensure they were aware of the situation. Her vigilance, quick thinking and dedication to maintaining a safe environment exemplified exceptional professionalism and teamwork.
